    This opportunity:

    Our organisation is currently seeking a professional and experienced Registered Nurse, to take on the role of Clinical Audit and Review Coordinator.  This role will see you working closely with our senior management team as you support the Resthaven Clinical Governance Model for both our Residential and Community services.

    A little more about the role:

    • Work in collaboration with the Senior Manager Clinical Services and other members of our clinical team as you support the Resthaven Clinical Governance Model
    • Plan, manage and report upon relevant projects, including making recommendations as required
    • Oversee the Mandatory Aged Care Quality Indicator Program and ensure all legislative reporting requirements are met
    • Oversight of the corporate clinical audit system and processes
    • Undertake clinical reviews as required
    • Facilitate staff education as required

    About you:

    • Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency is essential
    • Your knowledge of and commitment to risk management, continuous improvement and quality management systems is sound
    • You possess well developed conceptual and strategic thinking skills
    • Demonstrated ability to work in a changing environment, maintain flexibility and manage competing tasks and associated deadlines
    • Experience and demonstrated ability to act strategically, and set realistic goals and plans to improve service quality
    • Your communication skills are outstanding and you have the ability to communicate effectively with a wide range of stakeholders
    • The ability to analyse statistical data and report upon outcomes competently
    • A comprehensive knowledge of relevant legislation applicable to Aged Care services
    • Knowledge of clinical governance systems and processes
    • A current drivers licence

    This opportunity:

    This part time contract position to 30 June 2020 works in accordance with Resthaven philosophy and policies to work in collaboration with Advanced Practice Nurse Dementia, Mental Health and Program Managers, to undertake research into defining a Model of Care for Resthaven Residential Aged Care Services.

    Recommendations for the Model of Care will be congruent with the strategic objectives of Resthaven Inc. and some out of hours work may be required.

    A little more about the role:

    Undertake duties in accordance with the philosophy, business practices and policies of Resthaven Incorporated, and perform the following duties:

    • Work in collaboration with Advance Practice Nurse Dementia and Mental Health in progressing the key strategic project related to development of a Model of Care to enhance delivery of care and services provided within Resthaven services.
    • Plan and manage the project including development of timelines, key outcomes and milestones.
    • Provide project reports and recommendations as required on a regular basis.

    RAS Home Support Team Leader

    This opportunity:

    The Agedcare Alternatives Regional Assessment Service operates under contract from the Australian Government to undertake intake and eligibility assessment, case management and linkage services for older people who are seeking access to the Commonwealth Home Support Programme. We are seeking applicants with experience in the aged care community service sector in South Australia to work in our Agedcare Alternatives Regional Assessment Service on a full time basis.

    A little more about the role:

    • Develop service plans and work instructions appropriate to the delivery of support in the home for older people requiring either ongoing low intensity services or short term higher intensity services in the community.
    • Coordinate and oversee the activities of Home Support Assessors, and undertake more complex assessments using the National Screening and Assessment Framework
    • Assess individual personal and social needs of clients who are eligible to receive Commonwealth Home Support Programme assistance
    • Develop Support Plans that promote wellness and independence and express these using a re-ablement and/or self-management approach.
    • Develop appropriate service responses based on older people’s preferences and their assessed need, including identifying appropriate referral pathways
    • Assist the Manager with the allocation of assessment case-loads, rosters and supervision of staff involved in the provision of Regional Assessment Services
    • Ensure equitable access and culturally sensitive delivery to people of non-English speaking or Aboriginal / Torres Strait Islander backgrounds

    About you:

    • Applicants must be credentialed as a Home Support Assessor and should include copies of their Statement of Attainment Certificate from Canberra Institute of Technology (CIT). It is essential that you have your own appropriate vehicle with driver’s licence.
    • You will have current registration as RN or Allied Health Professional with AHPRA.
    • Knowledge of aged care related policy issues.
    • Experienced in aged care as a health professional.
    • Demonstrated ability to facilitate older people designing their preferred support plan.

     The Regional Assessment Service operates under contract from the Commonwealth Government to undertake intake and eligibility assessment, case Management and linkage services for older people who are seeking access to the Commonwealth Home Support Programme.

    Reporting to the Manager Agedcare Alternatives Regional Assessment Service you will be responsible for managing the work flow and day to day clerical operations of the program.
